2020 Small Group Application

The Illinois Department of Insurance requires carriers to file enrollment applications each year. Health Alliance introduced the 2021 small group enrollment application in January 2021. We understand you have many competing priorities so we will continue to accept the 2020 small group enrollment application through 5/31/2021. However, because we are required to file the enrollment application each year, we cannot accept the 2020 small group enrollment application after 5/31/2021.You can access the 2021 small group enrollment application from the employer portal or by emailing ClientSupport@healthalliance.org and they will email a copy.
